About Helping Hands Aftercare

Helping Hands Aftercare, located in theAtlanta GA is a support program geared towards supporting individuals with lifestyle choices, changes, and challenges. Group classes focus on the seven critical self-care behaviors identified to best produce good outcomes by the American Association of Diabetes Educators (AADE). Helping Hands Aftercare Program is for adults, age 18 and over who primarily have type 2 diabetes and stroke.
Group instruction includes education about:
- • being active ( Lifesteps, a required exercise program is used)
- • healthy eating
- • taking medication
- • monitoring blood glucose levels
- • problem solving
- • reducing risks of diabetes complications
- • healthy coping strategies
